SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
Assign the principal diagnosis, secondary diagnosis and procedure codes for inpatient discharges,
Ambulatory Surgery and ER visits, utilizing 3M HDM coding system, EPIC Electronic Medical Record.
Report Congenital Malformation to the State.
Analyze the discharged patient's medical record for completeness and accuracy, to ensure that they are
following hospital, corporate and outside Government Agencies standards.
Generate queries for documentation specialist for query follow-up.
Interact with Clinical Documentation Specialists for query follow up.
Maintains confidentiality of information from patient's medical records.
Performs concurrent and retrospective clinical documentation review and provides data when necessary.
Works with other departments to ensure that accurate reporting and reimbursement are facilitated.
Perform other duties as assigned by Manager / Director.
